I Yudi M am writing this letter now that I have been trying to correct my daughters name now that on her birth certificate it appears as, Danna Gabriela Caracas when it should have been Danna Gabriela M. I have been to Vital Statistics and the hospital where she was born but still I have not been able to fix this problem. My daughter is underage, therefore she
cannot present herself before a judge, and this is why I am asking Vital Statistic to help me, now that they are the only one that could help me.
I have come across many problems now that I cannot apply for her passport and other documents due to the fact that her name has been mistaken. This process has taken a lot of my time away, my time is very valuable. I need Vital Statistics to straighten this issue or I will be forced to send this case to the Supreme Court. I am not asking for any substantial
money, the only thing I am asking is Vital Statistics to arrange this so I can quickly obtain her correct birth certificate, obtain her passport, social security and also fix her school information.
