Anyone receiving request form for double refund of unauthorized charges to your credit card

My credit card was charged over six times in two years for so-called "membership fees" which I did NOT authorize, and I finally received a refund for all.

Now I have received a snail mail form from Galleria asking me if I wish to request a DOUBLE REFUND if Galleria cannot produce a telephone recording proving that I DID request the membership charges!!!

I find it hard to trust that they would truly provide double credit. I hope they are not just collecting names and addresses and phone numbers for whatever OTHER dubious reasons...

Anyone else get this form??
