Similar thing happened to me, except I was applying for medicare benefits by the instruction of the hospital. The hospital also told me not to respond to Collections since I was dealing directly with the debtor. Weeks passed as I was getting my documentation together and these calls and letters without identifying information kept arriving. I reported them to the Federal Trade Commission who does nothing, absolutely nothing except assign you a complaint number. Everything else in the prior complaint-same thing happened to me.
You should ask the hospital to get that removed from your credit report—people wonder why the U.S. Is 42nd in healthcare and probably #1 in debt, look at the system. The government doesn't protect its citizens against predatory corporations who violate the law every day. I limit my interaction with any services unless needed like a cell phone, etc. If I am ever sick again I'm not going to a hospital unless I have insurance—what a country.
