This report is true. I have witnessed racial favortism shown to black individuals. There is improper documentation of medication forms and dispensing of medications is sometimes skipped or documented with incorrect numbers. The forms are legal documents, yet they are sloppily written on and illedigible, as well as written on the back. They are ripped, torn and folded.
