My account was debt for 99.49 without my permission

I spoke to a representative and was told to come to this site and get a cancellation form and fax back to them with a bank statement and will be givin my money backand insufficent fees. At this point I donot see a cancellation form. This was unauthorizes and cause my bank account to go into over draft.
