Claimed to be official business name renewal reminder. Charged $125

Received an official looking letter (with a form to fill out) stating my Business Name is about to expire, and requesting payment by a certain date. Being extremely busy, I filled out the form and sent a check for $125 without really checking it out. (Stupid, I know!)

I learned my lesson and want my money back!
