Received letter from SAIC, dated Nov 5, claiming to be a government contractor supporting the TRICARE Management Activity (TVA). The letter states that on Sep 14, a SAIC employee reported that computer backup tapes containing my information were stolen from his vehicle in San Antono, TX. Backing up the infomation to these tapes and transporting them for storage in a remote location is a routine procedure to save important data and is a specific contract requirement for SAIC. The letter goes on to say that upon disovery of the theft, they promptly notified law enforcement and designated government agency. They did not identify the government agency. The letter goes on to say that the information contained on the tapes relates to laboratory workups sent to San Antoniomilitary treatment facilities for processing and includes names, Social Security Numbers, addresses, dates of birth, phone numbers, laboratory tests, provider names and provider locations, but does not include any financial data such as a credit card or bank account information.

There is more info in the letter which I am prepared to FAX if desired. Attached to the letter is a Consumer Credit Report and Credit Monitoring Auhorization Form which asks for Social Security Number, Date of Birth and Telephone Number and the company offers FREE Credit Monitoring for a year.
