Failed to pay on insurance policy on cell phone replacement

My name is Charles and I filed a claim with Asurion for property that was stolen (cell phone) on April I was given a proof of loss application to fax in to 866-306-2268, I was told I needed to get it notarized and a police report in which I did but no one has gotten back to my on my $200 metro cell phone back on 09-26-08.

I sent the manugacturer UTStarcom and model number COM-8932 serial number 008000431021 the cell was stolen out of QT station in Sandy Springs I got a police report 59669.in May of this year I faxed again to the office of "Asurion" Claims Documentation Processing 648 Grassmere Park Dr Ste. 200 Nashville TN 37211 and still no replacement cell phone.

I had insurance on the phone as of May company never replaced it. I ended up going to another cell carrier but my money was paid for that service under its insurance.
