Charged my bank account for $20.95 on 9/25/07 without my approval Belle Vernon

I was going through my monthly charges on my bank statement and came across a $20.95 on 9/25/07 to LSM5-us-pay.com for which I DID NOT give my permission or approval. I would like to report a FRAUDULENT report on this company charging customers (myself) for charges not approved. I think this is outrageous and should be stopped!!!
