Home Design fraudulent billing, false fax number given, said not recieving info sent to them Ripoff

I have phoned serveral times to clear bill that was paid off. They keep saying they have not recieved info. Hve faxed info to them at least six times since October. Mailed info twice. They continue to say they have not recieved payment when bank showed they cashed payment in December.
