Lost Money to Capial Choice Consumer Credit

I sent in my $39.00 by check and it cleared my bank account like a debit. That should have been my first clue. Actually, my first clue should have been a credit card that didn't need to do a credit check. There is no phone number on the application (which I still have). Only an address:

I received a "shopping" credit card in the mail but didn't realize the two were related and have since thrown it away and all paperwork that went with it.
