Like others I see here on this site I filled out this form thinking it was the Social Security Administration. It was not until I received the typed form from Bennett & Gray that I realized my mistake. I don't care about the money that I wasted as much as I am concerned that I have given them alot of information about me. Social Security #, birthdate / place, and my debit card # to pay for it.
I cannot find a phone # anywhere for them. It is not on their letterhead, in information or online. I am so scared that i was so stupid. I am going to change my bank account, go to the BBB and hope they are not as shady as I fear.
