BSing me about my guaranteed refund

Garanteed me a complete refund if I cancelled before 30 days. I did and now they are telling me I have to complete some form they never sent me.

I don't want to crucify this company but I want my $298.95 back. They lie about everything I've talk to them about in hope I will commit more money to them. Please just refund my money and this whole process ends, continue and this is not the only website that deals with untruthful people
