I completed and signed the official Medical Records Release form and gave it back to the Hospital on 11/29. The following day (11/30), the Hospital confirmed that it had received my release form.
HealthPort mailed an envelope and it included six pages dated 12/5. HealthPort wrote that it is unable to comply with my request for medical records because HealthPort states the authorization was not signed by me, the patient (even though it was)! Here is a copy of the letter's text verbatim:

Dear patient:
We are unable to comply with your request for medical records for the following reason (s):
The authorization was not signed by the above mentioned patient. It is the Hospital and HIPPA policy to request supporting documents, in the event the patient is unable to sign for the release of their medical records.
