Civic Center Dental Associates in Scottsdale Arizona owes me $255.80. They have been aware of this since 1-6-00 and they haven't paid me my money back. My phone calls and visits haven't had any results. I have a computer printout that states that they owe me $126.69.in addition I have a cancelled check for $128.00 that they never gave me credit for. I have brought a copy
of this check to their office.
Please, help me get my money back!
