Collection Bill from AFNI for Carrier Account Never Opened

I received this bill from afni saying I owe 629.00 I have never had T-Mobile as my carrier. My husband also received one and my son did too. And they have never had T-Mobile as their provider. Please let me know something. What should I do? Thanks so much. Please reply.
