Unusual and excesive Credit Card Fee Ripoff

I requested a credit card and I received a first bill that includes the following charges:
Program Fee $95.00
Account Set-up Fee $29.00
Participation Fee $ 6.00
Annual Fee $48.00

Thus I have not made any purchases with the card but already owe this bank $178.00.!

I looked up the page enclosed with the application form that I sent and it says nothing about these fees except the following message: See enclosed insert for information about account fees, rates, costs, limitations available credit and other terms.

Yet, this "insert" is not clipped with the page that I saved, therefore I believe that they did not include it in the envelope. Otherwise I would have saved it with the page that I have in my records for this application.
