I first received a government-looking advertising form in the mail 2 weeks ago. The form is deceptive, as it has no information about any product or service, and no information about their company. Even the company email is not given.
At that moment, I was waiting for some important forms from the government. I do not know where the company stole my information. I thought it was one of the required forms from the govern, and signed my initial by mistake and sent the form back to the provided Washington DC address (their main branch is in New york).
After about one week, I received a CD in the mail. Since I did not order it, I threw it away as garbage.
Today I received a invoice which looks for a payment of $312.00 for something I do not want or ordered.
