Preferred Health Benefit
Took money out of my checking account, unauthorized, won't answer phone calls, won't refund money

This company receives personal information as a third party. They never call to offer services, but some how manage to sign you up and charge a fee of 49.95. A phone number was available on the check from their company (with my name on it), but no one ever answers, after numerous calls.
One person did answer after weeks of calling, I cancelled the service in time to receive a refund. It has been a month, where is my refund?
