Performed maintenance check on home a/c and and shortly thereafter disconnected wire caused system to go out

There was another report just like this on usacomplaints.com that helped me ascertain that the odds are high something is very fishy with Ace Cool Air. Our home A/C was working fine but we had Ace Cool Air conduct maintenance check just to make sure all was OK. They did the basic tune up and said everything was fine, but mentioned that the unit was old and they didn't know how long it would last. The next day we noticed it didn't seem to be running quite as cool (you think it would run better after the "tune up") and was a bit noisier, but it was working OK. About a week later it stopped working completely. After having the unit checked out (we figured why go with Ace again if it was working better before they did their maintenance service on it?), it was discovered that one of the power supply wires overheated and melted so as to break contact with where it was screwed in to the terminal connector... But more importantly it was noticed that the screw for the terminal contact was loose, and not screwed in the same as all the other ones, which had created a bad connection, heated up the wire and finally caused heat and melting that broke the contact. It was also suspected that this might have been why to A/C wasn't running well after the service visit.

Any good A/C technician checks all basic electrical connections to ensure that all the they are good, tightens up any that seem loose, and looks for any signs of arcing or insulation damage due to heating of the wiring. This wire had melted insulation and the copper had melted and broke away from the contact point. The screw was sticking out and was about 5 or 6 turns looser than any of the other exact same type of contact terminals/screws that used the same size wire. How could anyone miss this issue? Especially when it is supposed to be part of a service check up.

When I found someone else had the same problem mentioned on usacomplaints.com it made me think: Hmmmmmmmmmm... What are the odds of that happening again by accident???
