Refuses to come back and provide services already paid for

On March 30, I called and left a message for American Repair Services LLC inquiring about installing appliances and taking away the old appliances. Later, that same day, Matt Maquire called me back and quoted some prices and scheduled an appointment for the next afternoon, March, 31. No one showed.
On April 1, I called again, spoke with a woman, advised they were a no show and she scheduled an appointment for that afternoon. Around 3:30, Justin with American Appl, came and installed my stove and microwave. He then told me that he was running late and could he come back and hook up the ice maker hose, install the dishwasher and take away all the old appliances another time. I agreed and he then charged me exactly what had been quoted by Mr. Maquire for the appliances he installed. He then said the charge for the dishwasher and ice maker would be $93.90 for installation and parts (not including tax) and I paid that as well for a total of $387. Xx. He said he would be back the next week and I could call the office to schedule the date and time.
On April 8, I called and scheduled an appointment for Friday April 10. On April 10, they called and said Justin did not show up for work that day and they will have to reschedule me for Monday and that if Justin didn't show up on Monday, April 13, they would send someone else between 4 and 6.
On April 13, around 5 the office called me and said, "Matt is running late, he will not be able to make it by 6." I said that was fine, I will be home all evening. No one showed.
On April 14, I called again, advised they were a no show and scheduled an appointment for that evening. No one showed.
On April 15, I called again and now I am no longer pleasant. I told them I want the installation I paid for in advance performed on Saturday, April 19 or I want my money back. She assured me someone would be there on April 19 between 2 and 4.
At 3:50 on April 19, Matt Maquire called me himself and said they have no record of me or my address at all or of any services being provided or paid for and that "Justin" doesn't work for them and that he "stole your address and service request information from the internet" and came to my house to provide the services and he is the one that took my money and American Appliance has nothing to do with it and they are not coming to install or give me my money back.
Number 1, how exactly did this Justin get my information from the internet when I never put it on the internet. My initial contact with American Applicance was a phone call directly from Matt Maguire (who now says he has no record of me);
Number 2, if they have no record of my request, why did the receptionist continue to make appointments;
Number 3, if they have no record of my request, how did the receptionist have my phone number and repeatedly call advise they were running late, etc;
Number 4, I have an American Appliance Repair receipt for services, yet they say they have no record of me;
Number 5, my check has been cashed and cleared the bank, yet my dishwasher is still in a box in the garage as well as my old stove and microwave which they were to haul away.
