I am filing a report about a form I recieved in the mail, asking me to submit $125.00 to file my annual meeting minutes for my sub-contracting/supplier business located here in Southern Illinois. The name on the form was from:

The note read as follows: The preparation of annual meetings does not satisfy the requirement to file the annual report required bu Illinois Business Corporation Act 805 ILCS 4/14/05. The annual report and instructions may be found online.
However, the "instructions" were not online.
