CONCRETE DRIVEWAY AT MY HOUSE - Concrete Driveway

I hired Mr. David Sterba, owner of True North Cement Co. To pour a new Driveway at my House, located at 16873, Rabbit Run Dr., Strongsville, Ohio 44136 in June.in March of I notice that the Driveway is chipping off, causing pits on the surface. I called Mr. Sterba. He told me that he will fix it. He did not show up on the scheduled appointment. He blocked my phone number and won't accept my calls.
