I ordered a death certificate under the name of Serafin Julio Melegrito using a credit cardof Ms. Rona May A. Valles card no. (Personal Information Removed) on July 22 and up to now the requested death certificate did not received yet. The total amount i can pay was USD 70.00 and amounting 3,009.91 in peso that was deducted to my cardon that date i ordered. I registerd on CLKBANK*COM amounting 915.10 pesos before i requested to your office. Please response and give me a copy of the said request for military purposes tnx... ASAP response.
