Walmart has to know this scam is happening and it IS coming through Walmart. I filled out the survey with my most recent sales slip from Walmart. I am entitled to belive that my information submitted through this survey would not be used for scamming purposes. Walmart has the liabilty to ensure this information is not used for any other purpose other than their stated survey.
