Formsleader.com put a fraudulent $4.65 charge on my card on 12/28/09. I was first tipped off by the Fraud Department at my bank. I called the phone number provided on my statement and received a recording saying I should contact them by email. I did and received the same response others have received to the effect that they had already reversed the charge, etc. They, of course, have not reversed the charge. My bank advises that I must cancel the card in order to dispute the charge. That is what I plan to do.
