I was informed that I could have medical benefits for me and my family for $369.00 plus $125.00one time enrollment fee. They fradulently withdrew funds without providing the medical benefits

To whom it concerns:
I am writing this letter in reference to a withdrawal that was taking out of my checking account fraudulently on 11/20/08 by a Mr. Arnold Montanez from Medical & Health Solutions Inc. I was informed that I would receive medical benefits for me and my family for $369.00 plus a one time enrollment fee of $125.00; the ACH debit totaled $494.00. Each time I attempted to contact Mr. Montanez or the company it went straight to the answering machine. I researched this company and found out that this company has done this to other individuals just like me. I'm requesting that this ACH debit be reversed based on the fraudulent act that was committed on 11/20/08.
