I just rec'd a form w-2 wage and tax statement 2012 listed as control Number 5501013 from this company. I have never heard of them nor have I ever worked for them.
It is reported I had state of wages of $70.00, with ss withheld of $2.94 and medicare of $1.02 and CA Disa. 70
