My credit card information was taken from the computer and then items were charged on my checking account in my name

My debit/credit card information was taken from the computer and she charged up alot of money on it. She ordered flowers for her birthday and also ordered some other items from different online companies. I have her name, address and phone number and want everyone to know who she is. Now I am having a very hard time getting my money back and clearing my name.
