Ripped me off for $29,95 per month for the last 10 months never heard from them Utah nationwide

They charge me $29.95 per month. I have received nothing from them. I want there charges stopped and a refund of all monies they have stolen from my checking account. I have contacted the stste of florida and the federal gov. They want more info from me. I need phone #s, address, and who is in charge. Thanks
