Lied and misled in a successful attempt to steal money

My wife became active on my employer's health insurance January 1st. She has a prescription for birth control which is apparently covered by UHC. She is used to paying less than twenty dollars a month through her employer's provider and so was shocked when in the month of January she filled her prescription and was told she had to pay the full price of $93. We do not make much money and we are forced to scrimp and save to make ends meet. We did not have the money available so she did not fill her prescription and called UHC for an explanation. She was advised that she had a $100 annual deductible she had to meet before United Health Care would cover ANY of the prescription costs that we were paying every month for. She accepted this answer and we budgeted for the additional cost in February. She paid full price in February and then when she went back in March she was again advised she had to pay the full $93 cost. During the followup call to United Health Care she was advised that since we had only paid $93 of the $100 deductible we would again have to pay full price, thereby meeting the $100 deductible and all future refills would be covered. It strained us financially to do so, but we came up with the money and made it work, knowing we would be in a better financial situation the next month. My wife just went to fill her April prescription and was AGAIN charged $93, which we AGAIN do not have. During this followup call she was advised that she is not covered if she picks up the prescription in the pharmacy and that she has to purchase a 90 day supply from UHC and have it mailed to her, which no previous representative even hinted at. They claimed they never told her any of the things she was advised on the phone and that the prior conversations had never occured. When she asked if they keep notes in their system of previous conversations she was advised no and was told they also do not record phone calls and that there is no claims process available whereby we can get back the money that was stolen from us. How convenient. UHC intentionally, and with full knowledge of their actions, deceived us in order to avoid prescription plan payouts knowing that we would have no recourse to retrieve our hard earned money.
