Complaint,

I am writing to you to tell you about how my health has been affected By EdgePark Medical supply. Back in November. First they sent my doctor the paper work for the order and the paper work was lost "in the mail" so, he resubmitted it. Somewhere around Christmas I was call by Edge Park and told everything was approved. While discussing the details with Jennifer I found out that the order was incorrect the only item on the order was the insulin pump. When I contacted Medtronics I was clear I wanted the pump with the continuous glucose monitor. I informed them of the error and told them I would wait until the order was complete before they shipped anything. After that I did not hear from them for about a month when I called to find out why I had not been contacted and how the order was progressing. I was then told by Trent that there was additional paperwork needed from my doctor and they had been unable to contact him. So, I contacted him. He then contacted them and the paperwork was completed and sent back to Edge Park. Then Trent contacted me and told me that the papers my doctor supplied would not be accepted by my insurance company.

I was not believing what I was hearing so, I contacted my insurance company and they said the paperwork was fine I
just needed a doctor to sent them a pressurization form which they mailed me and I made an appointment that coasted me $191. Plus $25. Fuel as I live 50 miles from town. I also had the doctor fill out a second set of order forms. I faxed a copy to my insurance company and to Edge Park. Trent contacted me again and told me that this paperwork was not
going to be enough and he would not order until I did exactly what he wanted because it would be denied. This is now 4 months later I am in ketoacidosis, stressed beyond limits. I tell him I am very ill and if he does not submit my order I will hold him responsible for my health. I am not sure if he sent the papers or if it was the copy I sent to the insurance company but, Monday march 19th I get an approval letter. I call Edge park to confirm my order is being completed I am told by Ben that there is a $800.00 co-pay I gave him my credit card information he tells me the order is complete. The products are being shipped overnight and will be delivered on Wednesday march 21st and someone must be home
to sign for the package. I have to refuse an overtime shift to be home for the delivery. I work in a hospital laboratory and lost $600.00 for the 12 hour shift. The package never arrives I never got a phone call or an email nothing.

I call the next day. The lady I speak with tells me that they did not ship my order because they needed a verbal
confirmation from my insurance and they had received it and the order was being sent rush over night. Today I get a phone message by this Trent telling me that my order can not be sent because of the error made in the beginning that the continuous glucose monitor as of 2012 is considered an add-on and will coast me an additional $1500.00. Out of
pocket and I needed to sign additional paper work to agree to pay this extra money. I wrote him back telling him I was no longer am interested in doing business with him or edge Park. Now I have to start the process all over again with another company.

The employees at Edge Park Supply play with peoples health and appear to have fun doing it. This guy Trent
has been playing god with me from the beginning. My heath has suffered since Medtronic gave these people my contact information. Your company may want to reconsider having Edge Park Supply represent your products. I have never been treated with less dignity and respect by anyone especially a company that is suppose to be helping people with
disabilities and life threatening illness. Almost 5 months have pasted and I have nothing to show for my wasted effort but frustration.
