I had been educated by Walgreens Drug-Store that my entire life period medical advantages have now been refused by Sedgwick CMS, the insurance provider addressing La Unified Scool District.

Our Statements document # with Sedgwick is WC648024232LM.

I've named Sedgwick numerous occasions without any reaction.

I received my honor in 1996, VNO 0291672 &VNO 0291673.
