I can no longer see why medical insurance premiums rise

The following is our benefits summary. There is no reason given for not paying the claims, or if there is, it is because they claim to have not recieved our Pre-existing Condition Form, although our policy specifically states that pre-existing conditions are not excluded. Despite that, we have mailed and faxed the form, and have the fax confirmations, with the following result:
