Still sending bills after giving cancel notice

I was on the mailing list to receive their book-Health Breakthroughs 2008. I did not want the book and on their pre-mailing invoice, I wrote please cancel, I did not want the book... They sent the book anyway. I returned the book to them in the mail... For the past 2 months they have sent a bill for the book.
