Called and canceled three times, and still getting charged, ripoff

Each month they take out 14.95 from my account. I've called and canceled THREE times. Everytime I call back, the person I talk to recalls no record of a cancelation. The confirmation code they give you for the cancelation is futile. It's been about five months since I got this, and I don't know where else to turn.
