Mutual of Omaha cancellation, free look, claims and where to complain: what the company's own pages say (checked October 6, 2026)

This section sets out what Mutual of Omaha's own pages and the regulators' pages said when opened on October 6, 2026. The 25 posts about Mutual of Omaha on this site are dated 2008 to 2013, so today's products, phone lines and procedures may differ from those years.

Which company stands behind the policy. The Medicare supplement page says its policy forms are “underwritten by Omaha Supplemental Insurance Company, a Mutual of Omaha company.” Its life insurance pages say life policies “are underwritten by United of Omaha Life Insurance Company” (Companion Life Insurance Company in New York). The policy documents name the issuer, so check them before writing to or complaining about a particular company.

Phone and mail. The Contact Us page lists “Individual Policy Owner – Customer Service: 800-775-6000” and “Covered by Employer – Customer Service: 800-655-5142,” with the mailing address 3300 Mutual of Omaha Plaza, Omaha, Nebraska 68175. For a life insurance claim, the File a Claim page offers an online form and says “Call us for help at: 888-493-6902” (“Individual coverage only”) or, “If your policy is through your employer, call 1-800-775-8805.”

Cancelling or returning a Medicare supplement policy. The disclosure text on the Medicare supplement page says: “You may cancel your policy at any time by giving us written notice,” and “In the event of cancellation or death, we will promptly return the unearned portion of any premium paid.” It also says: “If, for any reason, you are not satisfied with your policy, you may return it to us or to your agent within 30 days of its delivery. We will then promptly refund all premiums paid, minus any claims paid.” The same text says a policy is renewed “by paying the premium within the 31-day grace period.” Coverage can vary by state, so the policy and outline of coverage you received control. The same page also says coverage cannot be cancelled by the company as long as premiums are paid on time and there has been no material misrepresentation.

Medicare's own guidance on cancelling and switching. Medicare.gov, opened on October 6, 2026, says: “If you want to cancel your Medigap policy, contact your insurance company.” Its page on switching policies describes a separate 30-day free look: when you get a new Medigap policy you have 30 days to decide whether to keep it, you should not cancel the first policy until you have decided, and you pay both premiums for the month you have both. Whether a newly issued policy can be returned within 30 days is stated in the company's disclosure text quoted above and in the policy itself. Medicare also says that if you drop a Medigap policy to join a Medicare Advantage Plan and later return to Original Medicare, you may not be able to get the same policy back.

Life insurance terms to read before relying on a payout. The whole life and final expense pages list policy forms that include a “Graded Benefit Policy,” and the page's exclusions text says the death benefit will not be paid for suicide “within two years from the date of issue (one year in ND and MN),” in which case the premiums paid are returned. This page did not open the policy form for the graded benefit, so what applies to a particular policy is in that policy.

If the company does not resolve it. Complaints about an insurance company or agent go to the state insurance department of the state where the policy was bought. The Nebraska Department of Insurance page says: “If you did not live in Nebraska when you purchased the policy, please contact the state in which you lived at the time of purchase,” and lists what it cannot do, including “Order the company to pay your claim or refund premium.” The same page lists the Nebraska Insurance Complaint Division at 877-564-7323 (toll-free in Nebraska) or 402-471-0888, email [email protected]. The NAIC publishes a directory of state insurance departments. For Medicare questions, Medicare.gov lists 1-800-MEDICARE and State Health Insurance Assistance Programs (SHIPs), which help people “File a complaint or an appeal.”

Regulator actions involving Mutual of Omaha companies (primary documents)

Each item below is taken from the regulator's own page or the order itself. Admission language differs by order and is quoted for each.

U.S. Department of Labor, September 29, 2023 (group life insurance, evidence of insurability). The Department announced a settlement agreement with United of Omaha Life Insurance Company. Its news release says an investigation by the Employee Benefits Security Administration “found United often accepted premiums for years without determining if insurability requirements were satisfied,” and that United then “would often deny claims for benefits on the grounds the company never received the participant's evidence of insurability.” Under the agreement, whose parties are Mutual of Omaha Insurance Company, United of Omaha Life Insurance Company and Companion Life Insurance Company, the companies have 90 days from first receiving a participant's premium to decide eligibility, and after that cannot deny a claim solely for reasons relating to evidence of insurability. The release says United told the Department it “has voluntarily reprocessed claims dating back to February 2018.” The agreement's No Admissions clause says the agreement is not to be “deemed to be, or offered or received as evidence of an admission by or on the part of the Companies of any wrongdoing, fault, or liability.” The Department's claims are described in the agreement as allegations. The agreement covers employer-sponsored plans under ERISA, not individually bought policies.

Connecticut Insurance Department, Mutual of Omaha Insurance Company (Docket MC 24-55, listed July 2, 2024). A market conduct examination of Connecticut life and health business for 2022 found “one (1) producer acting as agent of Respondent without required appointment.” In the Stipulation and Consent Order the company agreed to pay a fine of $1,500, and the order states that “Respondent admits to the allegations contained in paragraphs three and four of Article I.”

Connecticut Insurance Department, United of Omaha Life Insurance Company (Docket MC 25-75, listed January 5, 2026). An examination covering 2023 found “Fifteen (15) producers acting as agent of Respondent without required appointment.” The consent order imposes a fine of $22,500 and uses the same admission wording as the 2024 order.

Massachusetts Division of Banks, Mutual of Omaha Mortgage, Inc. (consent order dated April 14, 2025). This is a licensed mortgage lender and broker with a San Diego main office, named in the order as “Mutual of Omaha Mortgage, Inc.” The order says the Division alleges that a website the company operates through a wholly-owned subsidiary failed to make advertisements or paid content sufficiently visually separate from recommendations or educational content, that it supersedes a February 7, 2022 consent order, and that the company entered the agreement “solely for the purpose of settling this matter, and without admitting” (the displayed text continues beyond what this page reproduces). It is a mortgage matter, separate from the insurance companies above.

Mutual of Omaha: who the companies are

Mutual of Omaha Insurance Company is a Nebraska-domiciled insurer; the Connecticut Insurance Department's order gives its NAIC company code as 71412. Its website's Company Profile says that in 1909 the company, “then known as Mutual Benefit Health & Accident Association,” filed articles of incorporation with the Nebraska Insurance Department (a company statement), and that its 1939 headquarters at 33rd and Farnam streets in Omaha remains part of the home office complex. The Contact Us page gives the mailing address 3300 Mutual of Omaha Plaza, Omaha, Nebraska 68175.

The Financial Strength page says Mutual of Omaha Holding Company “has two primary insurance companies”: Mutual of Omaha Insurance Company (health products) and United of Omaha Life Insurance Company (health, life, annuities and retirement products). The Department of Labor release calls Mutual of Omaha Insurance Company United's parent and Companion Life Insurance Company United's subsidiary. The same page lists A+ ratings from S&P Global and A.M. Best and A1 from Moody's for Mutual of Omaha Insurance Company; these are the company's own presentation of third-party financial-strength ratings and say nothing about claim handling or customer service.

The company's newsroom, opened on October 6, 2026, listed news releases from September and October 2026 about executive appointments and a rating announcement; no acquisition, regulator order or product shutdown appeared there.

Court and settlement record

Jones v. Mutual of Omaha Insurance Company (D. Md., No. 1:22-cv-00905), putative TCPA class action. The plaintiff alleged that Mutual of Omaha “hired a third party to send pre-recorded telemarketing calls” to her cell phone and those of other class members. On November 7, 2022 the court's memorandum opinion denied Mutual of Omaha's motion to dismiss for lack of personal jurisdiction and for failure to state a claim, finding the plaintiff had stated a prima facie claim that the third party acted as Mutual of Omaha's agent under an actual-authority theory. That was a ruling on whether the case could proceed, not a finding that any call was unlawful. The CourtListener docket shows the case terminated on June 12, 2023; the copy of the docket reviewed does not show the closing entry, so how it ended is not stated here.

Skinner v. United of Omaha Life Insurance Co. (District Court of Douglas County, Nebraska, Case No. D01CI2400006396), data incident class settlement. The official settlement FAQ says the plaintiffs claim a targeted cyber-attack gave access to the defendant's systems and compromised personal information, defines the class as people whose information “may have been compromised” in an incident between April 21 and April 23, 2024, and says that the defendant “denies it is or can be held liable,” and that class members could claim up to $1,500 per person for documented losses and lost time or an alternative cash payment. The deadlines were May 29, 2025 (claims) and April 29, 2025 (exclusion and objection); the final approval hearing was set for June 25, 2025, and a case update dated October 29, 2025 says settlement payments and credit-monitoring codes began going out that day. The claim and exclusion deadlines have passed.
