Aspen Dental Management, Inc. is a Delaware corporation founded in 1998, and the California Attorney General counts more than a thousand offices nationwide operating under the Aspen Dental name. Aspen Dental's own website says the offices that carry its brand are independently owned and operated by licensed dentists.
The company has also faced repeated government action over how its offices are run and advertised. New York ended its investigation in 2015 with an Assurance of Discontinuance and a $450,000 civil penalty. Massachusetts ended its lawsuit in 2023 with a consent judgment of $3.5 million, including up to $750,000 for certain patients. California announced a proposed settlement in 2026 with $2 million in civil penalties and $300,000 in restitution for certain patients, subject to court approval. In none of these did the company admit the state's allegations. A 2015 federal class action was dismissed because the plaintiffs had not shown a concrete injury, and a 2025 website-tracking class settlement states that the company denies the claims. Consumer reviews and complaints describe disputes over billing, insurance, recommended treatment and dentures; those are individual accounts, not findings.

Complaint record: BBB, Trustpilot and this site's archive
BBB. BBB has no single company-wide file for Aspen Dental; it lists separate profiles for many individual offices, each with its own rating and complaint count. The profile for Aspen Dental at 281 Sanders Creek Pkwy, East Syracuse, NY shows a B+ rating, not BBB accredited, with 6 complaints filed in the last 3 years and 1 not resolved. Those figures describe that one listing and are not totals for the company. To see complaints about a particular office, search BBB for that office's town.
Trustpilot. The Trustpilot page for www.aspendental.com shows a TrustScore of 1.6 out of 5 from 1,137 reviews, 266 of them in the last 12 months. The profile is unclaimed, and Trustpilot notes that the company has not invited its customers to review, so reviews may not be representative; Trustpilot says it does not fact-check reviews.
This site's archive. There are 115 published complaint reports linked to this profile. They name Aspen Dental or one of its offices (for example "Aspen Dental - Concord, NH"). Nearly all date from 2006 to 2014, most from 2008 to 2012. They are consumers' own accounts, not verified findings. The most common subject is insurance billing, charges and refunds, followed by x-rays, deep cleanings and other recommended treatment that the writer felt was oversold, then the fit and cost of dentures or partials, then financing such as CareCredit or in-office payment plans. One further report, filed mainly against a collection agency over CareCredit financing for Aspen Dental dentures, is linked here as a related mention.

Submit a ComplaintAspen Dental complaints: where to take a billing, treatment or advertising problem
Start with the office, in writing. Raise the problem first with the office where you were treated. Write to the office manager and ask for an itemized statement, a copy of the treatment plan you signed, the estimate you were given and what was billed to your insurance, then compare them with your insurer's explanation of benefits. State exactly what you dispute, the amount, and what you want: a corrected bill, a refund, a remake or adjustment of dentures, or release of a deposit. Use the office address printed on your bill or shown on that office's page at aspendental.com.
Aspen Dental's own channels. If the office does not resolve it, Aspen Dental's contact page lists billing and payment questions at (833) 902-4848, customer service at (844) ASPEN-11, which is (844) 277-3611, Spanish-language customer service at (855) 442-7438, and the email [email protected].

The 2015 New York Assurance and the 2015 federal court opinion both describe those practices as owned by licensed dentists, not by Aspen Dental Management itself. Your records, consent forms and bills may therefore name a separate practice entity, usually a professional corporation or PLLC; use that name, and the treating dentist's name, in any complaint.
Get your records. Under the federal HIPAA privacy rule you have a right to a copy of your dental records and x-rays, and the practice generally must provide it within 30 days of your request, with one 30-day extension allowed. A copy helps if you want a second opinion on a recommended deep cleaning or other treatment.
If you paid with CareCredit or another credit card. CareCredit is a Synchrony Bank credit card. For a billing error, such as a charge for treatment you did not receive, write to the card issuer within 60 days after the first statement showing the charge was sent; the federal Fair Credit Billing Act then requires the issuer to investigate. Many CareCredit promotions are deferred-interest offers: Synchrony explains that interest builds up from the purchase date and is charged if the promotional balance is not paid off by the end of the promotional period, so check the end date on your statement. Complaints about a credit card or loan can also go to the CFPB at consumerfinance.gov/complaint or (855) 411-2372; the CFPB says most companies respond within 15 days.

Regulators. Your state's dental board licenses the dentist and handles complaints about the quality of care, unnecessary treatment or records. Advertising and billing practices go to your state attorney general's consumer protection office; in California the Attorney General asks consumers to report false or misleading advertising at oag.ca.gov/report. Insurance disputes go to your insurer and then your state insurance department. You can also report deceptive practices to the FTC at ReportFraud.ftc.gov and file a complaint with BBB against the specific office, which BBB forwards to the business.
Government actions: New York (2015), Massachusetts (2023) and California (2026)
New York Attorney General, Assurance of Discontinuance No. 15-103 (2015). The Attorney General's office stated that it had received over 300 consumer complaints since 2005 about experiences at Aspen Dental offices. It found that ADMI was not merely a back-end support organization: it said the practices in New York were subject to extensive involvement of ADMI, shared profits with ADMI and were marketed under the Aspen Dental name in a way that created an impression of common ownership, and that this violated New York law (corporate practice and fee-splitting rules). The document lists seven dental practices operating 40 Aspen Dental offices in New York at that time. The Assurance states: "ADMI neither admits nor denies the Attorney General's findings in Paragraphs 3 through 34." ADMI agreed to change its business practices, including revisions to statements on its website and in marketing, and to pay a $450,000 civil penalty (paragraph 121). ADMI's chief executive signed in June 2015.
Massachusetts Attorney General consent judgment (announced January 5, 2023). The Massachusetts Attorney General sued Aspen Dental Management in December 2021, alleging that it charged patients for services advertised as free, advertised that its offices worked with "all" insurance although they did not accept MassHealth, the state's Medicaid program, and broke assurances it had given the office in a 2014 settlement over similar conduct. Under a consent judgment entered in Suffolk Superior Court, the company agreed to pay $3.5 million, including up to $750,000 returned to patients who paid for new-patient exams, x-rays and oral cancer screenings. The judgment bars ads that misleadingly offer "free" services, claim the offices take "all" insurance, mention guarantees or denture prices, or promise "no hidden fees", and restricts communications about oral cancer screenings and debt collection. Trade press reported that the consent judgment is not an admission of liability and that Aspen Dental denied wrongdoing.

California Attorney General settlement (announced May 7, 2026). The California Attorney General announced a settlement with Aspen Dental Management, Inc. over alleged violations of California's ban on the corporate practice of dentistry and false and misleading advertising. Allegations in the release include that the company exceeded a support role by directing practice and management decisions, selected, staffed and advertised offices without clearly identifying an independent dentist-owner, paid hygienists $50 for each clear-aligner sale to a new patient and $100 for each sale to an existing patient, and published advertising with misleading testimonials and unclear pricing, and that some advertisements said offices worked with all insurance or no insurance although the offices did not accept state or federally funded insurance programs. Terms listed in the release include $2 million in civil penalties, $300,000 in restitution for certain patients, and injunctive terms, including a ban on service fees based on practice revenue and a written fee schedule for products and lab services. The proposed judgment also requires advertisements to identify the practice owner's name conspicuously. The release says the settlement is subject to court approval. The Stipulation for Entry of Final Judgment published by the Attorney General (electronically received May 4, 2026; Los Angeles County Superior Court No. 26STCV14023, People of the State of California v. Aspen Dental Management, Inc.) states: "Defendant does not admit any facts or legal claims alleged in the Complaint and is settling this matter for the sake of resolution." Under paragraph 16 of the proposed judgment, the $300,000 restitution goes to two groups of patients: those who, before the judgment, paid the full fee for a "Problem Focused Exam" although they otherwise qualified for a "Free Exam and X-ray for new patients without insurance" and had not already received an adjustment, refund or credit; and those who bought an electric toothbrush as part of a bundle, up to the amount of the toothbrush (not the whole bundle). Paragraph 17 requires the company, within 90 days after entry of judgment, to make all reasonable efforts to contact these patients, using their last-known contact details, and pay them directly. Until the court enters the judgment, these remain proposed terms.
Lawsuits: a 2015 dismissal and a 2025 privacy-tracking settlement
Treiber v. Aspen Dental Management, Inc., N.D.N.Y. No. 3:12-cv-1565. Eleven named patients, treated at offices in eleven states, sued on behalf of a putative class against ADMI, its chief executive Robert A. Fontana and several holding and investment entities, alleging that the business arrangement with the practices broke state corporate-practice laws, and sought injunctive relief and damages. On March 27, 2015 Judge David N. Hurd dismissed the Amended Complaint in its entirety for lack of subject matter jurisdiction, holding that none of the named plaintiffs had alleged a concrete and particularized injury, such as being billed improperly or receiving unnecessary treatment. The court did not reach the merits and denied leave to amend again. On January 6, 2016 the U.S. Court of Appeals for the Second Circuit affirmed in a summary order (No. 15-1506-cv, 635 F. App'x 1).

Website tracking technology class action. Plaintiffs filed putative class actions on February 20, 2024 in the Northern District of Illinois, consolidated as A.D., et al. v. Aspen Dental Management, Inc., No. 1:24-cv-01404, alleging that tracking technology on www.aspendental.com shared website users' data. The claims included the federal Electronic Communications Privacy Act, negligence and wiretap or medical-privacy statutes of Florida, California and Pennsylvania. The federal case was voluntarily dismissed without prejudice on February 4, 2025 and refiled on February 20, 2025 in the Circuit Court of Sangamon County, Illinois, as Donnelly, et al. v. Aspen Dental Management, Inc., No. 2025LA000036, where the parties signed a class settlement agreement. The agreement states that Aspen Dental Management "denies each and every one of the claims" and that the settlement is not an admission of wrongdoing.
The settlement has two groups. Group 1 covers people who made an online appointment from February 20, 2022 through June 1, 2023 (an estimated 621,371 class members), sharing pro rata a fund of $2,796,169.50. Group 2 covers online appointments from June 2, 2023 through January 1, 2025 (an estimated 1,624,996 class members), with $15.00 per valid claim from a fund of up to $15,673,220, reduced pro rata if the fund falls short. Both funds include attorneys' fees and costs, and Simpluris was named as settlement administrator. The deadline to file a claim, opt out or object was September 15, 2025, so new claims can no longer be filed.

Who Aspen Dental is: a support organization and the offices that use its name
Aspen Dental Management, Inc. (ADMI) is a Delaware corporation. The 2015 New York Assurance gives its principal place of business as 281 Sanders Creek Parkway, East Syracuse, New York, and BBB still lists that address. The California Attorney General's May 2026 release says that since its founding in 1998 Aspen Dental has grown to more than a thousand offices nationwide, entered California in 2019 and opened 19 offices there, and is owned by private equity firms. In December 2021 the parent company was renamed TAG - The Aspen Group, which also runs other health care brands such as ClearChoice and WellNow; TAG opened a corporate headquarters in Chicago in October 2021, according to trade press coverage.
In the 2015 federal opinion in Treiber, the court summarized the plaintiffs' allegations, taken as true for the motion, that Aspen provides marketing, accounting, scheduling, purchasing, insurance-processing and IT services to practices under business service agreements with their licensed-dentist owners, and described ADMI as wholly owned by ADMI Corporation, itself owned by ADMI Holdings L.P., with entities managed by Leonard Green & Partners holding a majority interest at that time. A June 13, 2017 American Securities release says funds managed by Ares Management, L.P. and Leonard Green & Partners, L.P. agreed to increase their equity ownership in ADMI, alongside ADMI's leadership team and funds affiliated with American Securities; terms were not disclosed.

The structure matters for complainants. In the 2015 New York Assurance, ADMI is described as providing business support to independently owned practices that use the Aspen Dental trade name, while the Attorney General found that the arrangement went beyond that in ways that violated New York law; ADMI neither admitted nor denied the findings. The California Attorney General made a similar allegation about California offices in 2026, which the company also did not admit.

Frequently Asked Questions
Who owns the dental offices that use the Aspen Dental name?
Under Aspen Dental's model, each office is owned by a licensed dentist, typically through a professional practice entity, while Aspen Dental Management, Inc. provides business management and administrative services under the Aspen Dental name. The 2015 New York Assurance describes the New York practices as independently owned by licensed dentists. The New York Attorney General found, and the California Attorney General alleges, that the company went beyond a support role; the company neither admitted nor denied the New York findings and did not admit the California allegations. Aspen Dental Management itself is owned by private equity firms and is part of TAG - The Aspen Group. Check the practice name on your own paperwork.
What did the New York Attorney General's 2015 Assurance with Aspen Dental require?
Assurance No. 15-103 records the Attorney General's findings that ADMI was involved in matters reserved to dentist-owned practices, shared in their profits and marketed them in a way that implied common ownership, in violation of New York law. ADMI neither admitted nor denied those findings, agreed to change its business practices and the statements on its website and in marketing, and agreed to pay a $450,000 civil penalty.
What is the 2026 California Attorney General settlement with Aspen Dental?
On May 7, 2026 the California Attorney General announced a settlement with Aspen Dental Management, Inc. over alleged violations of California's ban on the corporate practice of dentistry and misleading advertising. The release lists $2 million in penalties, $300,000 in restitution funds for certain patients and injunctive terms, and says the settlement remained subject to court approval. The published stipulation (L.A. Super. Ct. No. 26STCV14023) states that the defendant does not admit any facts or legal claims alleged in the complaint. The $300,000 is for patients who paid the full fee for a Problem Focused Exam while otherwise qualifying for a free new-patient exam and X-ray, and patients who bought an electric toothbrush in a bundle (up to the toothbrush amount). Under the proposed judgment the company must try to contact these patients and pay them directly within 90 days after the judgment is entered.
What is the Aspen Dental website tracking class action settlement?
A class action over tracking technology on www.aspendental.com was refiled in Sangamon County, Illinois (No. 2025LA000036) and resolved by a class settlement agreement that says Aspen Dental Management denies the claims. It covers two groups of people who made online appointments between February 20, 2022 and January 1, 2025: a Group 1 fund of $2,796,169.50 shared pro rata, and Group 2 payments of $15.00 per valid claim from a fund of up to $15,673,220. The claim deadline was September 15, 2025, so new claims are no longer accepted.
What happened to the 2012 Treiber class action against Aspen Dental?
It was dismissed. On March 27, 2015 the Northern District of New York dismissed the Amended Complaint in its entirety, ruling that the court lacked subject matter jurisdiction because the plaintiffs had not alleged a concrete and particularized injury. The court did not decide the claims on their merits, and the Second Circuit affirmed on January 6, 2016 in a summary order (No. 15-1506-cv).
What do BBB and Trustpilot show for Aspen Dental?
Trustpilot shows a TrustScore of 1.6 out of 5 from 1,137 reviews on an unclaimed profile, with a notice that reviews may not be representative. BBB has no single company-wide file: it lists many separate Aspen Dental office profiles, each with its own rating and complaints. The East Syracuse, NY listing shows a B+ rating, not BBB accredited, with 6 complaints in the last 3 years, 1 of them unresolved. These figures are not added together.
Where can I file a complaint about an Aspen Dental visit or bill?
Start in writing with the office where you were treated, using the practice name on your bill, and ask for an itemized statement and your signed treatment plan. Aspen Dental's contact page lists billing and payment questions at (833) 902-4848 and customer service at [email protected]. If that fails, complaints about the quality of care go to your state's dental board; advertising and billing practices to your state attorney general (in California, oag.ca.gov/report); insurance disputes to your insurer and then your state insurance department; and problems with CareCredit or another financing product to the card issuer and the CFPB at consumerfinance.gov/complaint. For a credit card billing error, write to the issuer within 60 days after the statement showing the charge was sent. You can also report to the FTC at ReportFraud.ftc.gov and to BBB for the specific office.
Are the Aspen Dental reviews on Trustpilot representative?
Not necessarily. Trustpilot's own notice on the profile says the company has not invited its customers to review, so reviews may not be representative, and Trustpilot says it does not fact-check reviews. The profile has not been claimed by Aspen Dental.
Does Aspen Dental take Medicaid?
Aspen Dental's FAQ page states that it does not accept Medicaid and that it accepts most other insurance plans. For patients without insurance, the same page points to third-party financing options and the Aspen Dental Savings Plan.
Which dental insurance does Aspen Dental accept?
Aspen Dental's insurance page says it accepts most dental insurance providers, including Aetna, Delta Dental and MetLife, and that acceptance varies by location. To check a plan before an appointment, its FAQ page says to email [email protected] with your insurance details or call your local office.
What is the Aspen Dental Savings Plan?
It is a plan for patients without dental insurance. Aspen Dental's page describes one annual fee starting at $49 a year, up to 30% off services, free exams and X-rays, no waiting periods, and no annual maximums or deductibles. Aspen Dental states that the plan is not insurance and refers to the plan terms and conditions.
Does Aspen Dental publish a price list?
Aspen Dental's website has a pricing page it calls a price list, with sections for denture prices, implant prices and services such as checkups, extractions, crowns and root canals. The page says prices vary with treatment, materials and individual needs, and that personalized treatment plans include cost breakdowns before care begins.
What are Aspen Dental office hours?
Hours depend on the office. Aspen Dental's locations page lists offices with their address and shows whether each is open and when it closes or opens, and its FAQ page says an office's phone number and hours are found by searching for the local office. After a procedure, outside office hours, the FAQ recommends urgent care or a physician, and 9-1-1 in an emergency.
