Offered $10,000 platinum pre-approved credit card if I would send them $37.00 by 5/16/12

I recieved in the mail an offer to get a new credit card. It was from Union Workers Credit Services in Dallas, Texas. They stated I would get a pre-approved platinum credit card with a $10,000 credit limit, and at 5% APR. I had to reply by 05/16 or the offer would expire. I also had to mail them by check or money order an annual membership fee of $37.00, which is fully refundable with my first purchase using my $10,000 credit limit. All I had to do is sign the Platinum Card Activation Form and send them the $37.00.
