Family Health Providers National Roadside Association Charged 42.00 on my checking account without any prior knowledge Nationwide

Simply this: I logged on to my online banking service, and discovered that 42.00 had been charged on my checking account without any knowledge what so ever. I did not order the service or even agree to the service. I cannot imagine how this happened. How is this possible?
