Harassing phone calls, bogus billing

I repeatedly get phone calls demanding payment after payment has been made, posted and paid. My wife and I never opened an SST card account, it was sold to them by providian. On average we don't get billing statements every other month or for two months in a row. When I try and call their customer service I either get no one or the run around and am treated very rudely. I have been paying the balance down so I can get rid of them all together and everytime the balance gets close to zero I suddenly and mysteriously get hit with a couple of late/no payment charges and additional interest charges. Then they raise the minimum payment amount without sending a bill or notice of such payment amount increase.
