Ripoff ACH debit transaction from bank account for 9.95

USA benefits has deduced 9.95 out of my checking account as an ACH debit. The bank said they have no information on this company other than the ACH #. I have to sign a form for the money to be returned to my account. Tried to contact USA Benefits at phone number but it has been disconnected.
