Premeirclub.com still taking money from my checking and I am going to file a forgery affadavit on them

I thought I was applying for a credit card to shop their warehouse. They started taking money from my checking account. They still are doing it. I am looking in to a forgery affadavit. They have caused my account to be overdrawn and in a mess.
