Inc. Ripoff dishonest fraudulent billing

I found this $14.10 charge on the phone bill, haven't really contacted anyone yet. Phone bill had the company name, and a telephone number on the bill. So I came on-line did a search. My jaw dropped when I read all these posts. So what are my steps to do? I am really confused on how this all works. How did this happen? How can they sneak their way onto my phone bill? What can we all do? All these reports, and yet this company is still committing fraud.
