After confirming my account cancelation, Midphase automatically withdrew funds from me two years in a row

In after less than a year of service, I contacted Midphase to pay my balance and cancel my account. I did both using the channels they provided and according to their specifications. More than a year later, I received a bill and automatic withdraw of my funds, by the Midphase billing department. I contacted them again and spoke to someone several times before I was informed that Midphase had made a mistake and they were crediting back the full amount withdrawn. He assured me my account was definitely canceled, everything was cleared up and I would never be billed again.

I asked him specifically, "So are you sure my account is canceled this time?"Are you sure this won't happen again?" To both questions he answered yes and assured me this was the last I'd be hearing from them.

Another year has past since then and I just received another annual bill from Midphase for an account I cancelled years ago. I also received notice that they will be automatically withdrawing these funds from my bank yet again. Unfortunately their billing department will be closed until after the automatic withdraw tomorrow.

After this happening two years in a row, I wonder what is going on over there, and how many people have been "mistakenly" ripped off by Midphase.
