On or about June 13 (from records available at the present time) I received a phone call from "Unified Services", Las Vegas, NV concerning gifts including a shopping spree, internet services, long distance service, a dietary supplement, and a three-day and two-nite vacation.
The call came from Max Johnson of Unified Services, Floor supervisor in charge, ID # 1103-US in the Processing Dept. The original call came from phone number 866-766-9616. I was given 866-904-7796 as a number to call them if I have questions.
The charges explained to me in the telephone conversations was to be $1.95 one-time only to cover the mailing of the information packet. When it was all said and done $149.80 was deducted from my banking account. There was no mention in the phone conversations or in the information packet about the need to opt out if desired. It was presented as "if you don't participate there will be no further charges". And, the $149.80 was not mentioned either.
