Sweepstakes letter claiming I won over $2.5 million

I received a letter from Financial Notification Advisers in Indianapolis, Indiana. On the letter, it claimed that I won a cash award total of over $2,536,092.23. They sent me a Rapid Reply form for me to sign my signature on the right hand side at the bottom of the form.
