Fraud or incompetent employee

I have been trying to apply a cell phone since June. I sent in a copy of food stamp card both time and I was told that they were not received. The second time I sent in the application with an enlarged copy of the food stamp card stapled to the application form and I was told that it was not received. I am wondering if someone deliberately threw them away to deny my case or this company is only a scam or the person who handled the case is totally incompetent and disrespect what she or he was doing?
