Coverage ID 751327782 thats the number they gave us. They state as the plan Administrator for your Accidental Death and Dismemberment insurance. They are sending us this notice to inform us that they were unable to collect premium due from our account. They want us to send them $67.50 for premium due date January 31. I am very upset with these people who think can pull the wool over my face. I hope they get caught and spend a long time behind BARS that would give them a long time to think of doing it again. SHAME on them
