I put in a request for an address checkon the postal form provided by the USPS - which waives the fee. I went to the post office and had a postage due for $1.09, In checking, the postage due was a postage and fees paid envelope from the post office with $1.09. It was this form, retuned by a post office (no return address - a violation of postal rules) with this form in it. I did not get my mail.
I refused the postage due, it is at the Waukesha PO - Patty was the clerk. It was addressed to Cook Law Office-PO Box 1405,53187. There is no reason (1) for the postage due on a postage and fees paid envelope and (2) the use of an extra large envelope.
